ProviderLAW's Model Assignment & UCC Lien Form   |  ProviderLAW   |   Version 35   |  Last Updated:  May 27, 2011

This document has moved!  For a FREE copy of this model form, visit Free Resources.

ProviderLAW’s Model Assignment & Lien Document has been researched, developed, and tested over a 15-year period of time.  Among other things, this document has been designed to work “hand-in-hand” with the nationwide UCC lien framework.  There are two types of cases where it is especially important that you, the health care provider, use a well-drafted Assignment & Lien Document: (1) all types of accident cases (automobile, worker’s compensation, etc), and (2) certain types of health insurance cases. If you follow a 5-step protocol when it comes to your Assignment & Lien, including obtaining a well-drafted document, you can drastically reduce many of the recurring payment problems that affect your practice in these scenarios.  If you do not follow the protocol, you will continue to experience considerable lost revenue, lost time, and lost energy.  This fact has been proven time and time again.  Now for the first time ever, ProviderLAW's Model Assignment & UCC Lien Document is available in Spanish!

ProviderLAW Paper on UCC Lien Law:  “Nationwide UCC Lien Statute Helps Providers to Collect in Trauma Cases…”   |  ProviderLAW   |  Last Updated:  February 11, 2011, 5:15pm EST

ProviderLAW's paper on the UCC Lien Law was first published on the Tipology web site.  Since then, the paper has been submitted to various health care groups and organizations for possible publication in various formats. In this article, Keith Pendleton encourages readers to verify the UCC Lien Law for themselves, and also to speak with corporate counsel on the topic.

"Providers and staff – there is, I believe, a highly-valuable body of law which can help you and your patients.  This body of law has been developed over the past sixty years.  In the last ten years, however, the law has been amended in every State to apply specifically to health care practitioners.  The law is so significant -- and actually has been so impactful -- that any creditor is encouraged to exercise care in the way it is used.  Please consider speaking with corporate counsel before incorporating it into your procedures...."
